Upon arrival, you will be greeted by our friendly dental staff. You will be asked to fill out any new patient forms and provide proof of any dental insurance you plan to use. To make the process even smoother, our New Patient forms can be filled out online before your visit if you provide us with your email address when booking your appointment.
